How do I get Senior Farmers' Market Vouchers?

Choose one of the following options:

• Complete the online eligibility form below

• Call 888-486-9545 to complete an eligibility form

• Download this form, fill it out, and send it to your local ADRC office (additional eligibility agreement forms in other languages can be found below)

Local ADRC-CW addresses can be found on our Contact Us page

Fill out the form below to receive your vouchers. 

• The Senior Farmers' Market vouchers are provided on a First-Come, First-Serve basis.

• Once the ADRC-CW receives your completed & electronically signed eligibility agreement form, the Senior Farmers' Market Nutrition Program vouchers will be reserved for you to pick up at your local ADRC office or mailed to the address provided.

• Please call the ADRC-CW at 888-486-9545 if the vouchers are not received within 2 weeks of submitting the eligibility agreement form.
