The Wood County Transportation Program coordinates volunteer drivers for the DAV van to transport veterans to any VA Clinic in Wisconsin. Services Veterans in Central Wisconsin. Riders must be ambulatory (able to walk). Special arrangements for Veterans in wheelchairs can be made by calling (608) 504-1313. Ride is free. Medical Transportation Management, Inc (MTM) is the non-emergency medical transportation manager, contracted by the State of Wisconsin, for those people on Forward Health or in a nursing home. MTM is not for individuals who are enrolled in Family Care. Call at least 2 days in advance to schedule a ride. You will need to provide your name, address, phone number, Forward Health member ID number, address & phone number of where you will be picked up and dropped off, appointment date & time, any special transportation needs, and the general reason for the appointment. The unique Demand/Response system provides you with curb to curb service each hour.
