CAMP AMERICAN LEGION

DISABILITY PARKING CARDS

Information about Handicap Parking Cards is available at the local DMV office during office hours, via the website, or by phone via the Statewide Information Center 24 hrs./day. People may also call for information about licensing for drivers with special medical or physical conditions. To apply for a Handicap Parking Card: *Get an application form from the DMV and have a physician fill it out *Return it to the local DMV office with the appropriate payment *There is a $3 service fee for permanent cards for temporary cards, there is a $6 fee for one temporary card, $12 for two temporary cards (plus the $3 service fee) The service fee can be avoided by mailing in application. Eligibilities: Application signed by medical personnel.

DISABLED AMERICAN VETERAN VAN SERVICE

The Wood County Transportation Program coordinates volunteer drivers for the DAV van to transport veterans to any VA Clinic in Wisconsin. Services Veterans in Central Wisconsin. Riders must be ambulatory (able to walk). Special arrangements for Veterans in wheelchairs can be made thru Portage County DAV Program by calling (715) 346-1401. Ride is free. Veterans using this service are ineligible for VA travel reimbursement. Rides are filled on a first come first serve basis.

FORWARD SERVICE CORPORATION

Wisconsin Works (W-2) offers a wide range of employment, training and support services to help eligible Wisconsin residents secure and maintain employment. Individuals must have minor children to qualify and be working less than 29 hours/week. Participants complete up to 40 hours/week in assigned activities to get a monthly payment. Emergency Assistance can be provided by Forward Services. Assistance that can be provided includes: Rent assistance if minor child is living in the home (once/year), Assistance with WPS disconnects if child under 5 is living in the home or there is a condition requiring electricity for medical equipment, Homelessness assistance if there is a minor child in the home or impending 5 day eviction, Gas vouchers for those already enrolled in the W-2 program. Foodshare and Employment Training (FSET) require the individual to be receiving Foodshare and there is no monthly payment.

MARATHON COUNTY TRANSPORTATION PROGRAM North Central Health Care

Door-through-door transportation provided to residents of Marathon County who are over 65 or developmentally disabled who reside outside of the Metroride paratransit area. We use volunteer drivers for ambulatory people and medical vans for those who are non-ambulatory. A co-pay is charged according to the mileage from the origination to the destination of the trip. Hours are M-F 8-4:30. Members of Community Care of Central WI should call their caseworker or if on Medical Assistance should refer their request to Medical Transportation Management, Inc (MTM) (866-907-1493) the transportation broker for Wisconsin.

MEDICAL TRANSPORTATION MANAGEMENT, INC (MTM)

Medical Transportation Management, Inc (MTM) is the non-emergency medical transportation manager, contracted by the State of Wisconsin, for those people on Forward Health or in a nursing home. MTM is not for individuals who are enrolled in Family Care. Call at least 2 days in advance to schedule a ride. You will need to provide your name, address, phone number, Forward Health member ID number, address & phone number of where you will be picked up and dropped off, appointment date & time, any special transportation needs, and the general reason for the appointment. MERRILL-GO-ROUND

The Merrill Transit System, Merrill-Go-Round, is a provider of public transportation within the growing 12 square mile area of the City of Merrill. The unique Demand/Response system provides you with curb to curb service each hour. We pick you up, wherever you are and deliver you wherever you want to go within the city limits-some restrictions apply. Open weekdays only and call 1 hr before you wish to ride. General cost – $2.00, Elderly and Disabled – $1.00 Elderly and Disabled passengers are required to register with the Transit Office to receive a reduced fare card. Metro Ride Paratransit Service provides non-emergency, specialized transportation for people who are physically or cognitively unable to use a standard transit bus, both ambulatory and non-ambulatory user must be certified by Metro Ride. If you call Metro-Ride at 715-842-9287 by 4:30 pm on the prior day to your trip, a bus will pick you up at your location with-in the Metro-Ride service area. Eligibilities: Must have a disability which prevents use of a standard transit bus and be within 3/4 mile of a bus route. Must be able to use curb-to-curb service. Application completed by applicant and verified by physician or other verifying agency, and certified by Metro Ride.

Wheels of Independence provides transportation for the elderly and handicapped individuals statewide. The program is not designed for severe emergencies. WOOD COUNTY TRANSPORTATION PROGRAM

The Wood County Transportation offers several different transportation opportunities for adults age 55 and older and disabled adults in Wood County. Bus Service: Transportation provided to meal sites, grocery stores, beauty shops, or medical appointments. Buses run for anyone 55 and older and adults with disabilities. Fees are $1.50 one way. Call by noon the day before. Transportation-Out of town medical appointments or social events: Volunteer drivers provide transportation for medical appointments or social events. Serves seniors 60 plus and fees vary by destination and purpose of ride. Please make reservations 5 days in advance. Riders must be ambulatory. This service is non-emergency and covers the State of Wisconsin for medical appointments. Bus Hours: Monday to Friday, 7:30 am – 9 am for fixed route client pick-ups 9:00 – 2:30 pm for elderly and disabled adult pick-ups 3 pm to 4:30 pm for fixed route client drop-offs.
