IRIS –INCLUDE, RESPECT, I SELF-DIRECT

Include, Respect, I Self-Direct is Wisconsin’s fully self-directed Medicaid waiver support program for adults with disabilities and long term care needs. In IRIS, people determine the combination of supports and services needed, including who will provide the services and when and how they will be provided. IRIS is built upon the belief that every person, regardless of age or ability, has the right to lead a meaningful life. Eligibility criteria applies, contact your local Aging & Disability Resource Center of Central Wisconsin for more information.

OMBUDSMAN PROGRAM

This program was established by the state legislature to investigate complaints from any person concerning improper treatment of elderly persons receiving long-term care in a Wisconsin nursing home, Community Based Residential Facility, or Adult Family Home licensed by the State of Wisconsin. The program also offers advocacy for Family Care participants age 60 and over statewide. The Ombudsman in this office will:*Be an advocate to help resolve any problem or dispute relating to long-term care.*Provides presentations on nursing home residents’ rights and other topics related to long-term care.

OMBUDSMAN PROGRAM -DRW

This program was developed by the state legislature to assist people experiencing problems with the Family Care or IRIS programs. Person’s must be age 18-59 and eligible for Family Care or IRIS. The ombudsman can provide information and education on consumer rights, negotiate, mediate, and investigate complaints, assist consumers in filing complaints and appeals, assist in filing for administrative hearings. No charge for these services.
