GUARDIANSHIP SUPPORT CENTER

The Wisconsin Guardianship Support Center (GSC) is a statewide resource that works with vulnerable adults, their families and friends, guardians and other legally-authorized representatives, and professionals working with vulnerable adults. Through a toll-free helpline, the GSC provides information on the following subjects: guardianships, powers of attorney other advance directives, and more. The helpline is open to all individuals.

HONORING CHOICES

Aspirus offers free appointments at multiple locations to help people begin the advance care planning process. This involves a) reflecting on your value, b) choosing a healthcare agent, c) exploring goals for treatment, and d) completing an advance directive. The advance directive tool used is a Durable Power of Attorney for Healthcare. Advance Directive forms are available at physician offices, Aspirus Wausau Hospital, and at www.aspirus.org under “Patient Resources” or by calling the Aspirus Advance Care Planning Warm Line at 715-843-1340 or toll-free 844-624-4793. INDIAN LAW OFFICE OF WISCONSIN JUDICARE

Services Offered: *Legal assistance and representation to Tribal members in areas of Indian law statewide. (For Tribal members with incomes not more than 200% of the federal poverty level.) *Native American Elders Outreach Project: Legal assistance and advocacy for Native American elders with income maintenance and health care issues. (For Wisconsin residents who are enrolled or enrollable members of any federally-recognized tribe, and age 55 or above.) Examples include public benefit appeals for: * SSI/Social Security * Food Stamps/FoodShare * Medicare and Medical Assistance * Indian Health Services * Veterans Benefits * Native American Wills*For Ho-Chunk Tribal members, income not more than 200% of the federal poverty level. JUDICARE

Judicare provides free legal help for eligible persons with low income. You can apply for Judicare services by phone or online at www.judicare.org.
