4U ADULT DAY SERVICE

Provides adult day services to adults 18 years and older who have developmental disabilities. 4U offers its members opportunity to choose their daily activities, a chance to be a part of the community and gives them a chance to try new things and discover new talents and joys. We provide physical activity, exercise, crafts, games, puzzles, social interactions, Community outings, etc. Drop-ins welcome half days, full days from 6:00 a.m.- 6:00 p.m. Monday – Friday.

Aurora Adult Day Services has helped individuals with developmental disabilities become an integral part of their community. This is achieved through the development of social and life skills, participation in community activities and events and recreational programs. ASPIRUS LANGLADE HOSPITAL ADULT DAY CARE

The Aspirus Langlade Hospital Adult Day Center is a community program designed to meet the needs of adults with functional impairments through an individual plan of care. Adult Day Services provides a community of peers, a circle of friends and a support network for persons who would otherwise be isolated at their home. It provides an opportunity to learn new skills, relearn old skills and to interact with others in a safe and caring environment. Medical and prescription assistance and blood sugar monitoring is also available.

COMPANION DAY SERVICES

Companion Day Services (CDS) is a Social Adult Day Service Program designed to meet the needs of:*Adults who are recovering from recent illness, injury or surgery.*Frail or socially isolated adults living at home, alone, or with a caregiver.*Adults experiencing a decrease in physical, mental, or social functioning (including development disabilities and memory loss).*Caregivers in need of respite Participant needs will be met through our comprehensive programs designed to:*Help delay the progress of a participant’s existing disability.*Preserve or improve the participant’s health and level of functioning.*Maintain a participant’s level for independence.*Establish an individual plan of care based on an assessment of personal needs and strengths.*Assist with activities of daily living and personal care provided in a safe and secure environment.*Provide nutritional meals and snacks.*Foster companionship through social interactions with other participants, children, students, volunteers, and staff.*Caregivers in need of respite from their caretaking responsibilities.

HOMME ADULT DAY CENTER

Offers daily programs for persons age 62 or older with Alzheimer’s disease, dementia or other frailties of aging to engage in meaningful social and recreational activities. State Licensed respite care center provides time out for caregivers of adults with Alzheimer’s and Parkinson’s disease and memory loss (dementia), so they can run errands, schedule appointments, or take time for themselves and the rest of their families. NORTH CENTRAL HEALTH CARE – ANTIGO CENTER

Adult Day Service in Antigo primarily services individuals with Developmental Disabilities. The Adult Day Service encourages independence, daily living skills, socialization and community integration through a variety of activities. Consumers participate in regular activities that stimulate socialization, opportunities to develop relationships, self confidence and continued education development through tasks like sewing classes, arts and crafts, and exercise programs for example. The program currently runs Monday through Friday 8:00 a.m. to 4:00 p.m. We are also looking for caring and compassionate individuals to volunteer. If interested contact Jessica Pyke at 715-627-6635.

PARK PLACE – ADULT DAY SERVICES

Park Place Adult Day Services is a non-profit United Way supported agency providing supervision, socialization and physical assistance to dependent, vulnerable or isolated adults. We have wheelchair accessible showers and a monthly foot clinic. Our staff is specially trained for dementia care. We are VA approved.
